Financial Aid Reinstatement
If you have received notice from the Financial Aid office about being on financial aid disqualification and you would like to pursue reinstatement, please complete the Financial Aid Reinstatement form and return it to Director of Student Services, either in person in the Education Building Room 300 or by email.
Make sure to fill out sections A, B, C and D thoroughly before submitting the form. (Section E is for office use only). Provide supporting documentation as necessary. All financial aid appeals concerning your academic progress will be reviewed on an individual basis.
You will be notified of the reinstatement decision via email. It is important to note that your petition may or may not be approved.
